Roles & Responsibilities

• Provide compassionate, resident-centered nursing care to support recovery and quality of life
• Administer medications and treatments accurately and document per facility policy
• Monitor and record vital signs, changes in condition, and communicate concerns to the care team
• Assist with ADLs and ensure a safe, comfortable environment for each resident
• Collaborate with RNs, therapists, and interdisciplinary team members to develop and follow care plans
• Educate residents and families about care plans, medications, and recovery expectations
• Maintain accurate, timely charting in the facility’s documentation system
• Uphold infection control standards and facility policies at all times

Qualifications

• Current LPN license in Massachusetts (Licensed Practical Nurse) — required
• If applying as an RN candidate, a current RN license in Massachusetts is accepted (job posted for LPN/RN classifications)
• Current BLS/CPR certification required
• Comfortable working in a skilled nursing / rehab environment
• Strong communication and teamwork skills; reliability and professionalism for per diem coverage
• Prior experience in long-term care or rehab settings is preferred but not strictly required

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care, restorative nursing, and ADL support
• Vital signs assessment and early recognition of changes in condition
• Electronic medical record (EMR) charting and timely documentation
• Infection prevention, safety protocols, and resident rights
• Clear communication with residents, families, and interdisciplinary teams
