Roles & Responsibilities
- Provide compassionate, resident-centered nursing care to support recovery and quality of life
- Administer medications and treatments accurately and document per facility policy
- Monitor and record vital signs, changes in condition, and communicate concerns to the care team
- Assist with ADLs and ensure a safe, comfortable environment for each resident
- Collaborate with RNs, therapists, and interdisciplinary team members to develop and follow care plans
- Educate residents and families about care plans, medications, and recovery expectations
- Maintain accurate, timely charting in the facility’s documentation system
- Uphold infection control standards and facility policies at all times
Qualifications
- Current LPN license in Massachusetts (Licensed Practical Nurse) — required
- If applying as an RN candidate, a current RN license in Massachusetts is accepted (job posted for LPN/RN classifications)
- Current BLS/CPR certification required
- Comfortable working in a skilled nursing / rehab environment
- Strong communication and teamwork skills; reliability and professionalism for per diem coverage
- Prior experience in long-term care or rehab settings is preferred but not strictly required
Knowledge & Skills
- Medication administration and documentation best practices
- Basic wound care, restorative nursing, and ADL support
- Vital signs assessment and early recognition of changes in condition
- Electronic medical record (EMR) charting and timely documentation
- Infection prevention, safety protocols, and resident rights
- Clear communication with residents, families, and interdisciplinary teams
