Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in a skilled nursing setting
• Assess, monitor, and document patient conditions accurately and promptly
• Administer medications and treatments safely according to policies
• Collaborate with interdisciplinary team members to create and follow individualized care plans
• Educate patients and families about care plans, medications, and discharge instructions
• Maintain infection control standards and ensure a safe environment for patients and staff
• Respond to patient changes and escalating needs with sound clinical judgment

Qualifications

• Active LPN or RN license appropriate for practice in Massachusetts
• Current BLS/CPR certification or willingness to obtain prior to start
• Previous experience in long-term care, rehab, or skilled nursing preferred
• Excellent communication skills and a team-oriented mindset
• Ability to pass background check and required facility screenings

Education

• Graduate of an accredited LPN or RN nursing program (program type appropriate to licensure)

Knowledge & Skills

• Strong patient assessment and clinical observation skills
• Medication administration and documentation accuracy
• Familiarity with infection prevention and resident safety practices
• Comfortable using electronic medical records and charting systems
• Ability to prioritize tasks in a fast-paced care environment
• Collaborative approach to working with nursing assistants, therapists, and providers
