Roles & Responsibilities

• Provide compassionate, skilled nursing care to rehabilitation patients, supporting their recovery goals
• Administer IV medications, blood products, and perform lab draws safely and accurately
• Monitor and interpret telemetry to detect changes in patient condition
• Perform comprehensive head-to-toe assessments and document findings in Meditech Magic
• Deliver inhaler and breathing treatments and manage point-of-care testing (iSTAT, glucometer)
• Assist patients with mobility using patient lifts and follow safe transfer protocols
• Collaborate with the interdisciplinary team to implement individualized rehab care plans
• Educate patients and families about the rehabilitation process and discharge goals

Qualifications

• Active Registered Nurse (RN) license (state licensure or compact as applicable)
• Minimum 2 years of nursing experience (rehab/post-acute experience preferred)
• BLS certification required
• IV therapy experience and competency (IV-certified is a must for LPNs; RNs should be comfortable with IV meds and blood administration)
• Proven ability to read telemetry
• Familiarity with Meditech Magic or similar EMR systems
• CRRN certification preferred
• ACLS preferred (within 90 days if requested by the facility)

Education

• Active RN licensure (Associate or Diploma in Nursing acceptable; BSN welcomed if available)

Knowledge & Skills

• Telemetry monitoring and rhythm recognition
• IV therapy, blood administration, and safe medication administration
• Point-of-care testing: iSTAT and glucometer use and interpretation
• Skilled use of patient lifts and safe patient handling techniques
• Delivering inhaler and breathing treatments
• Conducting thorough head-to-toe assessments
• Documentation and charting in Meditech Magic
• Understanding of quality indicator scoring in a post-acute/rehab setting
