Roles & Responsibilities
- Provide compassionate, skilled nursing care to rehabilitation patients, supporting their recovery goals
- Administer IV medications, blood products, and perform lab draws safely and accurately
- Monitor and interpret telemetry to detect changes in patient condition
- Perform comprehensive head-to-toe assessments and document findings in Meditech Magic
- Deliver inhaler and breathing treatments and manage point-of-care testing (iSTAT, glucometer)
- Assist patients with mobility using patient lifts and follow safe transfer protocols
- Collaborate with the interdisciplinary team to implement individualized rehab care plans
- Educate patients and families about the rehabilitation process and discharge goals
Qualifications
- Active Registered Nurse (RN) license (state licensure or compact as applicable)
- Minimum 2 years of nursing experience (rehab/post-acute experience preferred)
- BLS certification required
- IV therapy experience and competency (IV-certified is a must for LPNs; RNs should be comfortable with IV meds and blood administration)
- Proven ability to read telemetry
- Familiarity with Meditech Magic or similar EMR systems
- CRRN certification preferred
- ACLS preferred (within 90 days if requested by the facility)
Education
- Active RN licensure (Associate or Diploma in Nursing acceptable; BSN welcomed if available)
Knowledge & Skills
- Telemetry monitoring and rhythm recognition
- IV therapy, blood administration, and safe medication administration
- Point-of-care testing: iSTAT and glucometer use and interpretation
- Skilled use of patient lifts and safe patient handling techniques
- Delivering inhaler and breathing treatments
- Conducting thorough head-to-toe assessments
- Documentation and charting in Meditech Magic
- Understanding of quality indicator scoring in a post-acute/rehab setting
