Roles & Responsibilities

• Provide compassionate direct patient care, ensuring safety and dignity for every resident
• Administer medications and treatments accurately and document care thoroughly
• Collaborate with the interdisciplinary team to develop and follow individualized care plans
• Monitor patient conditions and report changes promptly to nursing leadership
• Assist with activities of daily living while promoting patient independence and comfort
• Support admissions, discharges, and transfers with clear communication and organization
• Maintain accurate, timely charting and comply with facility policies and regulatory requirements

Qualifications

• Current Licensed Practical Nurse (LPN) license in New Mexico or compact equivalent
• Recent clinical experience in long-term care, skilled nursing, or similar setting preferred
• Basic Life Support (BLS) certification required or obtained upon hire
• Up-to-date immunizations and ability to pass a background and drug screen
• Strong communication skills and a patient-centered approach

Education

• Vocational nursing diploma or equivalent required; completion of an accredited LPN/LVN program

Knowledge & Skills

• Solid knowledge of medication administration and infection control practices
• Comfortable with electronic charting and clinical documentation
• Ability to prioritize tasks in a fast-paced environment and respond to changing patient needs
• Skilled in patient assessment, wound care basics, and IV/enteral care as applicable
• Team-oriented mindset with excellent bedside manner and critical thinking
