Roles & Responsibilities

• Provide compassionate, hands-on care to residents including assistance with activities of daily living
• Administer medications and document according to facility policy and scope of practice
• Monitor and record vital signs, report changes to the interdisciplinary team promptly
• Perform accurate and timely charting in the electronic health record
• Collaborate with nurses, CNAs, therapists and providers to deliver person-centered care
• Support care planning, participate in shift handoffs, and ensure continuity of care
• Promote resident safety, dignity, and independence through attentive care and advocacy

Qualifications

• Current and active LPN license (Licensed Practical Nurse) — required
• Basic Life Support (BLS/CPR) certification — current
• Graduate of an accredited practical nursing program or equivalent
• Ability to pass background check, drug screen, and required immunizations
• Comfortable working the 2:00 PM - 10:00 PM shift and collaborating in a team environment
• Strong communication skills and a patient-centered attitude

Education

• Graduate of an accredited Practical Nursing program (LPN diploma or certificate required)

Knowledge & Skills

• Medication administration and safe documentation practices
• Patient assessment and monitoring of changes in condition
• Skilled in wound care, ADL assistance, and basic clinical procedures appropriate to LPN scope
• Familiarity with electronic health record charting
• Excellent communication, time management, and teamwork skills
• Ability to prioritize tasks in a fast-paced long-term care setting
