Roles & Responsibilities

• Provide compassionate direct patient care and assist with daily living activities
• Administer medications and treatments accurately and according to orders
• Monitor patient status, document changes, and communicate concerns to the care team
• Support wound care, dressing changes, and other skilled nursing tasks as needed
• Collaborate with RNs, physicians, and support staff to coordinate safe, effective care
• Educate patients and families with clear, kind communication to promote recovery and comfort
• Maintain accurate charts in the electronic health record and ensure compliance with facility policies

Qualifications

• Current Licensed Practical Nurse (LPN) license required
• BLS/CPR certification preferred or obtain upon hire
• Previous experience in long term care or skilled nursing is a strong plus
• Comfortable with medication administration, wound care, and charting
• Strong communication skills and a team-oriented attitude

Education

• Practical Nursing diploma or equivalent vocational nursing certificate

• Medication administration and documentation
• Basic patient assessment and vital signs monitoring
• Wound care and common nursing procedures
• Electronic health record (EHR) charting
• Infection control and HIPAA compliance
• Clear verbal and written communication with patients and team members
