Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents consistent with the plan of care
• Administer medications and treatments safely and accurately, following facility policies
• Perform timely and thorough documentation in the electronic health record
• Monitor patient status and communicate changes to the interdisciplinary team
• Support admissions, discharges, and transitions of care with a patient-centered approach
• Assist with wound care, ADLs, and therapeutic interventions as assigned
• Participate in quality improvement activities and help maintain a safe environment

Qualifications

• Current LPN license in New Mexico or eligible to obtain licensure
• Minimum clinical experience as an LPN preferred (exact years may vary by facility)
• Current BLS (Basic Life Support) certification required
• Ability to pass background check and required health screenings
• Strong communication skills and a collaborative, patient-first attitude

Education

• LPN diploma, certificate, or equivalent completion of an accredited practical nursing program

Knowledge & Skills

• Skilled in medication administration and basic clinical procedures
• Comfortable with electronic health record documentation
• Knowledge of infection control standards and patient safety practices
• Solid assessment skills and the ability to escalate changes appropriately
• Good time management and ability to prioritize tasks in a fast-paced setting
• Team-oriented mindset with strong interpersonal skills
