Roles & Responsibilities

• Provide compassionate, direct patient care with attention to safety and dignity
• Administer medications and treatments according to plan and facility policies
• Perform accurate patient assessments and communicate changes promptly
• Document care thoroughly and maintain clear, professional records
• Collaborate with nurses, providers and support staff to deliver coordinated care
• Educate patients and families on care plans, medications and discharge instructions
• Contribute to a positive unit culture by mentoring others and promoting teamwork

Qualifications

• Active Licensed Practical Nurse (LPN) license (required)
• Current BLS/CPR certification (or ability to obtain upon hire)
• Comfortable with medication administration, patient assessments, and documentation
• Strong communication skills and a team-first attitude
• Clinical experience in a long-term care or skilled nursing environment preferred

Education

• Practical Nursing diploma or equivalent vocational nursing program completion

Knowledge & Skills

• Medication administration and basic IV/med pass familiarity
• Patient assessment and vital signs monitoring
• Electronic charting and accurate documentation
• Infection control and safe patient handling practices
• Effective communication with patients, families and interdisciplinary teams
• Time management and prioritization in a fast-paced environment
