Roles & Responsibilities

• Provide compassionate direct patient care during the 2:00 PM - 10:00 PM shift, including vital signs, ADLs, and medication administration
• Monitor patients’ conditions and document findings accurately and promptly in the patient record
• Collaborate with RNs and the interdisciplinary team to implement and update individualized care plans
• Perform wound care, dressing changes, and other treatments as ordered to promote healing
• Educate patients and families on care plans, medications, and safe transitions
• Maintain infection control, safety standards, and a clean, therapeutic environment
• Support a positive patient experience by delivering respectful, patient-centered care

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Mexico
• Current Basic Life Support (BLS) certification preferred
• Comfortable working as a travel/contract LPN and adapting to new facility routines
• Strong communication skills and a team-first attitude
• Experience in long-term care or similar settings is a plus

Education

• Completion of an accredited practical nursing program (LPN diploma or certificate)

Knowledge & Skills

• Medication administration and accurate documentation
• Basic wound care and dressing change techniques
• Proficient in taking and interpreting vital signs and patient assessments
• Familiarity with electronic medical records (EMR) charting
• Patient education, safety practices, and infection prevention
• Ability to prioritize tasks and collaborate with interdisciplinary teams
