Roles & Responsibilities

• Provide compassionate direct patient care including bathing, dressing, and mobility assistance
• Administer medications and document medication administration accurately under scope of practice
• Perform routine assessments and monitor vital signs to detect patient changes early
• Collaborate with RNs and interdisciplinary team members to implement individualized care plans
• Maintain clear, timely documentation in the patient record and communicate updates during handoff
• Support wound care, IV site observations, and other delegated clinical tasks as directed
• Educate patients and families on care plans, medications, and discharge instructions
• Champion patient safety and infection control practices at all times

Qualifications

• Current LPN license in New Mexico (or compact/state equivalent) — required
• Current BLS/CPR certification — required or obtain upon hire
• Previous experience in long-term care, skilled nursing, or similar setting preferred
• Comfortable with electronic medical records and clinical documentation
• Strong communication skills and ability to work collaboratively with a multidisciplinary team
• Ability to pass required background check and drug screening per facility policy

Education

• Graduate of an accredited LPN/Vocational Nursing program or equivalent

Knowledge & Skills

• Solid knowledge of basic nursing principles and bedside clinical skills
• Medication administration and safe documentation practices
• Patient assessment: recognizing changes and escalating appropriately
• Infection prevention, wound care basics, and safe patient handling techniques
• Familiarity with EMR systems and accurate charting habits
• Good time management, prioritization, and teamwork abilities
