Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a long-term care setting
• Administer medications and treatments accurately and document in the patient record
• Monitor patient status, identify changes, and escalate concerns to the care team
• Perform wound care, basic assessments, and routine clinical procedures
• Collaborate with nurses, providers, and support staff to create person-centered care plans
• Educate residents and families about care plans, medications, and discharge instructions
• Contribute to a safe, clean, and therapeutic environment that respects resident dignity

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Mexico or eligibility to obtain state licensure
• Basic Life Support (BLS) certification required; other certifications a plus
• Prior experience in long-term care, skilled nursing, or related clinical settings preferred
• Strong clinical judgment, reliable documentation skills, and a team-first attitude
• Ability to pass facility credentialing and background checks

Education

• Diploma or certificate in Practical Nursing (LPN) or equivalent vocational training

Knowledge & Skills

• Medication administration and basic clinical procedures
• Familiarity with electronic health record documentation
• Skilled in resident assessment, infection control, and wound care basics
• Clear communication with patients, families, and interdisciplinary teams
• Time management and the ability to prioritize in a busy care environment
