Roles & Responsibilities

• Provide compassionate bedside care and support to residents at Casa Del Sol Center
• Administer medications and treatments safely and accurately following facility protocols
• Monitor and document patient status, vital signs and responses to interventions
• Collaborate with the interdisciplinary team to develop and follow individualized care plans
• Educate residents and families on care plans, medications and health maintenance
• Maintain accurate charting in the electronic health record and communicate changes promptly
• Participate in quality improvement initiatives and maintain compliance with facility standards

Qualifications

• Current Licensed Practical Nurse (LPN) license in New Mexico or eligibility to obtain one
• Active BLS/CPR certification or willingness to obtain upon hire
• Comfortable with medication administration, basic wound care and patient assessments
• Excellent communication skills and ability to work well with a team
• Ability to pass background check and any required health screenings

Education

• Completion of an accredited LPN/Vocational Nursing program
• High school diploma or equivalent

Knowledge & Skills

• Strong clinical assessment and basic nursing skills
• Familiarity with medication administration and documentation best practices
• Experience with electronic health records and accurate charting
• Good time management and ability to prioritize patient needs
• Empathy, good communication and patient-centered care approach
