Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications and treatments safely and document accurately
• Monitor and record vital signs, reporting changes to the clinical team
• Assist with admissions, discharges, and transfers to ensure smooth transitions
• Communicate clearly with patients, families, and interdisciplinary staff to support care plans
• Maintain thorough, timely charting in the electronic health record
• Follow infection control and safety protocols to protect patients and staff
• Support quality improvement initiatives to enhance patient outcomes

Qualifications

• Current LPN license (Licensed Practical Nurse) — New Mexico licensure preferred
• Active BLS/CPR certification (or willingness to obtain upon hire)
• Previous LPN experience in long-term care, skilled nursing, or similar setting preferred
• Comfortable working evening shifts and collaborating with a diverse care team
• Strong communication skills, professionalism, and a patient-centered attitude

Education

• Graduate of an accredited Practical Nursing program (LPN/LVN diploma or certificate)

Knowledge & Skills

• Solid knowledge of basic nursing assessments and medication administration
• Charting and documentation best practices (electronic health record experience a plus)
• Ability to prioritize, multitask, and manage time during busy shifts
• Good clinical judgment and the ability to escalate concerns promptly
• Teamwork, empathy, and clear patient/family communication
