Roles & Responsibilities

• Provide compassionate direct care to residents, focusing on dignity and comfort
• Administer medications and treatments accurately and document per facility policy
• Perform routine assessments and promptly report changes in condition
• Collaborate with the interdisciplinary team to create and follow individualized care plans
• Assist with activities of daily living and promote patient independence
• Maintain accurate, timely documentation in the patient chart
• Support admissions, transfers and discharges to ensure smooth transitions

Qualifications

• Active LPN license in New Mexico or eligibility to obtain one quickly
• Completion of an accredited practical nursing program
• Basic Life Support (BLS) certification required or must obtain upon hire
• Comfortable working in long-term care or skilled nursing settings (experience preferred)
• Ability to pass background check, drug screen and any required immunizations
• Strong communication skills and a team-first attitude

Education

• Graduate of an accredited Practical Nursing (LPN/LVN) program

Knowledge & Skills

• Solid knowledge of medication administration and charting practices
• Skilled in wound care basics and infection control principles
• Patient assessment and reporting — know when to escalate
• Familiarity with electronic health records (EHR) and documentation standards
• Time management and prioritization in a busy clinical setting
• Customer-service mindset with residents and families
