Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents
• Administer medications and treatments safely and accurately
• Perform routine assessments and monitor changes in condition
• Document care clearly and timely in the electronic health record
• Collaborate with nurses, therapists, and providers to coordinate care
• Educate residents and families about care plans and safety measures
• Maintain infection control and safety standards at all times
• Support admissions, discharges, and transitions with professionalism

Qualifications

• Required: Licensed Practical Nurse (LPN) - active and in good standing
• Current Basic Life Support (BLS) certification preferred
• Comfortable working in a long-term care or skilled nursing environment
• Strong communication skills and a patient-first attitude
• Ability to pass required background and health screenings

Education

• Graduate of an accredited practical nursing program or equivalent diploma

Knowledge & Skills

• Skilled in medication administration and basic nursing procedures
• Proficient with patient assessments and vital sign monitoring
• Accurate clinical documentation and familiarity with EHR systems
• Knowledge of infection control, safety protocols, and wound care basics
• Excellent teamwork, time management, and prioritization skills
• Empathy, cultural sensitivity, and strong bedside manner
