Roles & Responsibilities

• Provide high-quality, compassionate direct patient care to residents at Casa Del Sol Center
• Administer medications and treatments safely and accurately according to policies
• Monitor and document patient status, changes, and interventions in health records
• Collaborate with nurses, therapists, and support staff to create and follow individualized care plans
• Educate patients and families on care plans, medications, and discharge instructions
• Maintain a safe, clean care environment while following infection control protocols
• Participate in shift handoffs and communicate critical information clearly

Qualifications

• Current LPN license in good standing (New Mexico licensure preferred or compact if applicable)
• Basic Life Support (BLS) certification required; additional certifications welcomed
• Previous experience in long-term care, skilled nursing, or similar settings preferred
• Current immunizations and the ability to pass background and health screenings
• Strong communication, organization, and teamwork skills
• Ability to work the evening shift (2:00 PM - 10:00 PM) reliably

Education

• Completion of an accredited practical nursing program (LPN diploma or certificate)

Knowledge & Skills

• Skilled in medication administration and documentation
• Knowledge of vital sign monitoring and basic assessments
• Comfortable with electronic health record charting and care documentation
• Good clinical judgment, prioritization, and time management
• Strong interpersonal skills for patient and family communication
• Ability to work collaboratively within an interdisciplinary care team
