Roles & Responsibilities

• Provide compassionate direct patient care including medication administration and wound care
• Monitor patient status, record observations, and report changes to the care team
• Collaborate with RNs, physicians, and interdisciplinary staff to coordinate care
• Document accurately in the chart and maintain timely, thorough clinical records
• Assist patients with activities of daily living and promote patient dignity and comfort
• Educate patients and families about care plans, medications, and follow-up needs
• Adhere to infection control policies and facility safety standards at all times

Qualifications

• Active LPN license in good standing (required)
• Current Basic Life Support (BLS) certification or willingness to obtain prior to start
• Previous experience in long-term care or skilled nursing preferred
• Up-to-date immunizations and ability to pass background and drug screening
• Strong communication skills and a team-oriented attitude
• Flexibility to work evenings and adapt to the needs of a travel assignment

Education

• Vocational Nursing diploma / LPN program graduate

Knowledge & Skills

• Solid medication administration and documentation practices
• Basic wound care and pressure injury prevention
• Patient assessment and reporting vital changes to the care team
• Familiarity with electronic health record systems and accurate charting
• Time management, prioritization, and ability to work independently when needed
• Compassionate bedside manner and family communication skills
