Roles & Responsibilities

• Provide compassionate bedside care and daily support to residents
• Administer medications and treatments accurately and document thoroughly
• Perform focused assessments and escalate changes in condition to the RN or provider
• Collaborate with the care team to create and follow individualized care plans
• Assist with activities of daily living while promoting resident dignity and independence
• Maintain clear, timely charting and support smooth shift handoffs
• Support infection control practices and maintain a safe environment for residents and staff

Qualifications

• Valid Licensed Practical Nurse (LPN) license in New Mexico or eligibility to obtain one
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Clinical experience in long-term care, skilled nursing or similar setting preferred
• Ability to pass background checks and meet facility immunization requirements
• Strong communication skills and a team-oriented mindset

Education

• Graduate of an accredited LPN/LVN program or equivalent diploma/certificate

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and catheter management
• Resident assessment and recognizing acute changes in condition
• Electronic medical record (EMR) familiarity and accurate charting
• Patient-centered care, empathy and de-escalation techniques
• Time management and prioritization in a busy care environment
