Roles & Responsibilities

• Provide compassionate bedside care to residents, maintaining dignity and comfort
• Administer medications and treatments safely and according to policy
• Assess and monitor patient conditions; report changes to the interdisciplinary team
• Document care accurately and thoroughly in the medical record
• Support patient and family education, offering clear instructions and reassurance
• Collaborate with nurses, therapists, and support staff to create and follow care plans
• Assist with admissions, discharges, and transitions to ensure continuity of care

Qualifications

• Current, active Licensed Practical Nurse (LPN) license (New Mexico or compact where applicable)
• Basic Life Support (BLS/CPR) certification required or must obtain upon hire
• Experience in long-term care, skilled nursing, or similar settings preferred
• Strong communication skills and ability to work as part of a multidisciplinary team
• Ability to pass facility-required background checks and health screenings

Education

• Graduate of an accredited practical nursing program (LPN diploma or certificate)

Knowledge & Skills

• Proficient in medication administration and documentation
• Skilled in basic wound care, vital signs, and patient assessments
• Comfortable with electronic medical records and accurate charting
• Knowledgeable about infection control practices and safety protocols
• Ability to prioritize tasks in a fast-paced clinical environment
