Roles & Responsibilities

• Provide compassionate direct patient care to residents in a skilled nursing setting
• Administer medications and treatments accurately and safely per provider orders
• Monitor and document patient status, vital signs, and changes in condition
• Coordinate care with nurses, therapists, and interdisciplinary team members
• Assist patients with activities of daily living and ensure comfort and dignity
• Maintain clear, timely electronic medical record documentation and handoffs
• Follow infection prevention, safety protocols, and facility policies
• Educate patients and families about care plans and post-discharge needs

Qualifications

• Valid LPN license in New Mexico or eligibility to obtain state licensure
• Current BLS/CPR certification or willingness to obtain prior to start
• Previous experience in skilled nursing, long-term care, or similar setting is preferred
• Ability to work evening (2:00 PM - 10:00 PM) shifts and adapt to facility scheduling
• Successful completion of background checks and required health screenings

Education

• Graduation from an accredited Practical Nursing (LPN/LVN) program

Knowledge & Skills

• Medication administration and safe documentation practices
• Basic clinical assessment and monitoring of vital signs
• Knowledge of wound care and common nursing interventions
• Strong communication, teamwork, and resident-centered care approach
• Familiarity with electronic medical records and charting
• Infection control practices and adherence to safety protocols
