Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents at Harbor Hill Center
• Perform thorough assessments and document changes in condition accurately in the chart
• Administer medications and treatments safely, following facility protocols
• Collaborate with the interdisciplinary team to develop and execute individualized care plans
• Educate residents and families on care plans and health maintenance in a respectful way
• Support a safe environment by following infection control and safety procedures

Qualifications

• Active LPN license in good standing (required); RN candidates considered as indicated
• Current Basic Life Support (BLS/CPR) certification preferred or willing to obtain
• Valid professional credentials and ability to pass background and health screenings
• Comfortable working 12+ hour day shifts and managing clinical priorities
• Strong communication skills and a collaborative approach to patient care

Education

• LPN: Graduate of an accredited practical nursing program
• RN applicants: ADN or BSN from an accredited nursing program if applying as RN

Knowledge & Skills

• Medication administration and safe documentation practices
• Skilled in patient assessments, wound care basics and vital sign monitoring
• Familiarity with electronic medical records and accurate charting
• Ability to prioritize, multitask, and respond calmly in urgent situations
• Strong interpersonal skills for working with residents, families and care teams
