Roles & Responsibilities

• Deliver compassionate direct patient care including medication administration and basic nursing treatments
• Monitor and document patient status and communicate changes to the interdisciplinary team
• Assist with ADLs (activities of daily living) and provide comfort measures to promote recovery
• Perform wound care, IV care per facility policy, and other delegated nursing tasks
• Support care planning by providing accurate, timely charting and handoffs
• Collaborate with nurses, providers, and support staff to ensure safe, high-quality care

Qualifications

• Current LPN license in New Mexico or eligible to obtain state licensure
• Graduate of an accredited practical nursing program
• Basic Life Support (BLS) certification required; ACLS or PALS as applicable is a plus
• Prior experience in long-term care, assisted living, or similar setting preferred
• Strong clinical assessment, communication, and teamwork skills

Education

• Practical Nursing diploma or certificate from an accredited program
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and infection control procedures
• Patient assessment and reporting clinical changes accurately
• Familiarity with electronic medical records and bedside charting
• Excellent communication, compassion, and time-management skills
