Roles & Responsibilities

• Provide compassionate direct patient care, including medication administration and monitoring
• Perform routine assessments and accurately document changes in condition
• Collaborate with RN and interdisciplinary teams to create and follow care plans
• Assist with ADLs and promote patient independence and dignity
• Communicate clearly with patients, families, and staff to ensure continuity of care
• Participate in infection control practices and maintain a safe care environment
• Support admission, discharge, and transfer processes to keep the unit running smoothly

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Mexico or eligible for state licensure
• Basic Life Support (BLS) certification required; additional certifications a plus
• Comfortable working in long-term care or similar settings (experience preferred)
• Strong clinical judgment, reliable attendance, and a team-oriented approach
• Ability to complete facility onboarding and required compliance training

Education

• Completed accredited Practical Nursing program or equivalent
• High school diploma or GED

Knowledge & Skills

• Medication administration and accurate charting in electronic health records
• Routine nursing assessments, vital signs, and wound care basics
• Familiarity with long-term care workflows and regulations
• Effective verbal and written communication with patients and providers
• Time management, prioritization, and teamwork under a busy schedule
