Roles & Responsibilities

• Provide compassionate, direct patient care to residents at Casa Del Sol Center
• Administer medications and treatments safely and according to provider orders
• Monitor and document patient status changes, vitals, and responses to treatment
• Collaborate with nurses, therapists, and support staff to create great care plans
• Maintain accurate, timely documentation in the patient record
• Educate patients and families about care plans and medications with patience and clarity
• Follow infection prevention, safety protocols, and facility policies to protect patients and staff

Qualifications

• Active Licensed Practical Nurse (LPN) license — New Mexico license required or compact privilege if applicable
• Current Basic Life Support (BLS) certification preferred or required per facility policy
• Previous LPN clinical experience in long-term care or skilled nursing preferred
• Comfortable with medication administration, basic wound care, and routine assessments
• Ability to pass background check and drug screen as required by the facility

Education

• Graduate of an accredited Practical Nursing (LPN/LVN) program or equivalent vocational nursing diploma

Knowledge & Skills

• Strong clinical assessment skills and attention to detail
• Medication administration and documentation accuracy
• Clear, compassionate communication with patients and families
• Teamwork and collaboration within interdisciplinary teams
• Time management and the ability to prioritize patient needs
• Basic electronic charting / EHR familiarity
