Roles & Responsibilities

• Provide compassionate assistance with activities of daily living — bathing, dressing, grooming and toileting
• Take and accurately record vital signs; promptly report changes to the nursing team
• Assist with safe transfers, ambulation and repositioning to maintain mobility and prevent injury
• Document resident care clearly and timely in the charting system
• Collaborate with nurses, therapists and the care team to deliver coordinated care plans
• Maintain a clean, safe environment and follow infection control practices
• Support residents’ emotional and social needs with patience and respect
• Participate in effective shift handoffs and communicate concerns for continuity of care

Qualifications

• Valid CNA certification in Pennsylvania (or ability to obtain state certification prior to start)
• Completion of an accredited CNA training program
• Current BLS/CPR certification (or willingness to obtain)
• Up-to-date immunizations and TB screening as required by facility
• Successful background check and drug screen as part of onboarding
• Comfort with patient transfers and basic lifting; ability to follow safe patient handling procedures
• Strong communication skills, a team-oriented attitude, and genuine empathy for residents

Education

• High school diploma or equivalent
• Completion of an accredited CNA program and current state CNA certification

Knowledge & Skills

• Hands-on experience with ADLs and resident personal care
• Ability to take and interpret vital signs and recognize red flags
• Basic charting and documentation practices
• Infection control procedures and standard precautions
• Safe transfer and mobility assistance techniques
• Time management in a fast-paced care environment
• Clear, compassionate communication with residents and families
• Team collaboration and following care plans
