Roles & Responsibilities

• Provide compassionate personal care to residents, assisting with activities of daily living
• Support patient mobility, transfers, and safe positioning to reduce risk
• Take and record vital signs accurately and report changes to the nursing team
• Document care and observations clearly in the patient record
• Assist with feeding, hygiene, and comfort measures while preserving dignity
• Communicate promptly and kindly with patients, families, and teammates
• Follow infection control and safety protocols to keep everyone protected

Qualifications

• Current CNA certification in Pennsylvania, active and in good standing
• Minimum hands-on experience in long term care or acute care preferred
• Basic Life Support (BLS/CPR) certification preferred or willingness to obtain
• Proof of required immunizations and TB screening per facility policy
• Positive attitude, strong teamwork skills, and a patient-centered approach

Education

• High school diploma or GED required
• Certified Nursing Assistant (CNA) credential required

Knowledge & Skills

• Skilled in resident care and activities of daily living
• Comfortable taking and documenting vital signs accurately
• Familiarity with safe transfer techniques and mobility assistance
• Basic documentation skills, attention to detail, and time management
• Strong communication and empathy when interacting with patients and families
• Ability to follow care plans and collaborate with the interdisciplinary team
