Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents at Casa Del Sol Center
• Administer medications and treatments accurately and on time
• Assess and monitor patient condition, documenting changes in the care plan
• Collaborate with nurses, therapists, and physicians to deliver coordinated care
• Educate patients and families about care plans and post-discharge instructions
• Maintain infection control standards and promote a safe environment
• Support admissions, transfers, and discharge processes with clear communication

Qualifications

• Current Licensed Practical Nurse (LPN) license (required)
• Basic Life Support (BLS) certification or willingness to obtain
• Clinical experience in long-term care or similar settings preferred
• Ability to pass background check and drug screening as required by facility
• Strong interpersonal skills and a team-first attitude

Education

• Graduate of an accredited Practical Nursing program
• Active LPN licensure

Knowledge & Skills

• Medication administration and documentation
• Patient assessment and vital sign monitoring
• Electronic medical record (EMR) familiarity
• Infection control practices and safety protocols
• Clear communication with patients, families, and care teams
• Time management and prioritization in a fast-paced environment
