Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents
• Administer medications and document administration accurately
• Perform routine assessments and promptly report changes in condition
• Collaborate with RNs, physicians, and the care team to implement care plans
• Assist with treatments, wound care, and ADLs as assigned
• Educate patients and families on care, medications, and follow-up
• Maintain accurate medical records and comply with facility policies
• Contribute to a positive, team-oriented environment and support quality initiatives

Qualifications

• Current and unencumbered LPN license in New Mexico (or eligible to obtain)
• Basic Life Support (BLS) certification required
• Current immunizations and background checks as required by facility
• Previous experience in long-term care, skilled nursing, or similar setting preferred
• Strong clinical judgment, attention to detail, and excellent communication skills

Education

• Completion of an accredited practical nursing (LPN) program
• Diploma or certificate in Practical Nursing

Knowledge & Skills

• Solid knowledge of medication administration and documentation best practices
• Comfortable with resident assessment and care planning
• Familiarity with infection control and safety protocols
• Experience using electronic medical records (EMR) systems
• Excellent teamwork, organization, and time-management skills
• Ability to prioritize tasks in a fast-paced care environment
