Roles & Responsibilities

• Provide compassionate bedside care and support daily living needs
• Administer medications and treatments accurately and timely
• Monitor and document patient status, including vital signs and observations
• Collaborate with nurses, therapists, and the care team to deliver coordinated care
• Educate patients and families about care plans and promote patient safety
• Maintain accurate and timely electronic records and charting
• Follow infection control, safety protocols, and facility policies

Qualifications

• Current LPN license in good standing (Licensed Practical Nurse)
• Completion of an accredited LPN/LVN program
• Basic Life Support (BLS) certification preferred or willing to obtain
• Comfortable working in long-term care or skilled nursing environments
• Strong communication skills and a team-first mindset
• Ability to pass standard background check and drug screening

Education

• Graduate of an accredited LPN/LVN program
• State licensure as an LPN/LVN (required)

Knowledge & Skills

• Medication administration and documentation
• Patient assessment and monitoring of vital signs
• Basic wound care and ADL support
• Proficiency with electronic health records and charting
• Infection control and safety best practices
• Effective communication with patients, families, and teammates
• Time management and prioritization in a fast-paced setting
