Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients at Harbor Hill Center
• Administer medications and treatments accurately and document care in the medical record
• Assess patient condition regularly and communicate changes to the care team promptly
• Collaborate with interdisciplinary team members to create and follow individualized care plans
• Support admissions, discharges and transfers with efficient, patient-centered coordination
• Maintain a safe, clean environment and model best practices in infection control

Qualifications

• Active Licensed Practical Nurse (LPN) license—LPN or RN clinicians will be considered
• Previous clinical experience in long-term care or acute care is a plus
• Strong communication skills and a patient-first attitude
• Ability to work the posted day shift hours and collaborate with the care team
• Willingness to complete facility onboarding and any required training

Education

• Graduate of an accredited LPN program
• High school diploma or equivalent

Knowledge & Skills

• Solid nursing assessment and medication administration skills
• Experience with patient documentation and basic computer skills
• Familiarity with infection prevention and patient safety practices
• Team-oriented approach and ability to prioritize care in a busy setting
• Comfortable providing hands-on care to older adults and medically complex patients
