Roles & Responsibilities

• Provide compassionate direct patient care, including medication administration and timely assessments
• Monitor and document vital signs, changes in condition, and care provided in the patient record
• Collaborate with nurses and interdisciplinary teams to coordinate patient-centered care
• Perform wound care dressing changes and basic clinical treatments as ordered
• Support patients with activities of daily living while promoting independence and dignity
• Communicate clearly and professionally with patients, families, and the care team
• Follow infection control protocols and maintain a safe clinical environment

Qualifications

• Current Licensed Practical Nurse (LPN) license (eligible to practice in New Mexico preferred)
• Basic Life Support (BLS/CPR) certification required
• Completion of an accredited practical nursing program (diploma or certificate)
• Current immunizations and TB screening per facility policy
• Prior LPN clinical experience preferred; strong clinical judgment and patient-focus required
• Ability to pass background check and drug screening

Education

• Diploma or certificate from an accredited practical nursing program (LPN)

Knowledge & Skills

• Proficient in medication administration and documentation
• Skilled in taking and interpreting vital signs and performing routine assessments
• Good charting habits and familiarity with electronic health records (EHR) workflows
• Strong infection control and safety practices
• Excellent communication, teamwork, and time management skills
