Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents, focusing on safety and dignity
- Administer medications and treatments accurately and document all care in a timely manner
- Perform skilled nursing procedures within LPN scope, including wound care and basic assessments
- Collaborate with RN team members and interdisciplinary staff to develop and follow individualized care plans
- Communicate clearly and professionally with residents, families, and healthcare team members
- Monitor resident status, recognize changes, and escalate concerns to the appropriate clinician
- Contribute to a positive, resident-centered environment through teamwork and empathy
Qualifications
- Current, active LPN license (Licensed Practical Nurse) — LPN or T-LPN acceptable
- Ability to obtain and maintain any state-specific licensure required for West Virginia
- Demonstrated clinical skills in long-term or skilled nursing care (travel experience a plus)
- Strong communication and teamwork skills with a patient-first mindset
- Willingness to pass facility background checks and meet orientation requirements
Education
- Graduate of an accredited practical nursing program or equivalent vocational training
Knowledge & Skills
- Solid knowledge of medication administration and documentation best practices
- Basic assessment skills and ability to recognize changes in resident condition
- Comfort with electronic health records and thorough charting
- Good infection control practices and safe patient handling techniques
- Customer-service minded approach to resident and family interactions
