Roles & Responsibilities
- Provide compassionate bedside care to residents as a licensed LPN
- Administer medications and treatments accurately and safely
- Monitor and document vital signs, changes in condition, and care given
- Perform focused assessments and report findings to the care team
- Follow infection control protocols and maintain a safe environment
- Support nursing assessments, care plans, and interdisciplinary communication
- Complete Trinity specialty competency testing and facility-required documentation
- Participate in resident-centered care, promoting dignity and independence
Qualifications
- Current, active LPN license in Michigan (or eligible to obtain)
- Basic Life Support (BLS) certification required
- Successful License/Certificate Verification as part of onboarding
- Willingness and ability to complete Trinity specialty competency testing
- Must be able to complete required health screenings: fit test, 2-step PPD or IGRA, Trinity 10-panel drug screen, respirator evaluation, color vision test, COVID questionnaire
- Up-to-date immunizations as listed (COVID-19, MMR, Hepatitis B, Influenza, Varicella, Tdap) or documentation of immunity
- Physical/health statement clearance prior to start
- Previous experience in long-term care or skilled nursing preferred but not required
Education
- Graduate of an accredited LPN/Vocational Nursing program or equivalent
Knowledge & Skills
- Strong clinical skills in medication administration and wound care
- Solid charting and documentation skills — concise and accurate
- Ability to work well within an interdisciplinary care team
- Good communication and patient/family interaction skills
- Familiarity with infection prevention practices and PPE use
- Comfortable with competency testing and facility-specific protocols
