Roles & Responsibilities
- Provide compassionate, patient-centered hospice and long-term care in the home setting
- Perform thorough nursing assessments and develop individualized care plans
- Administer medications and treatments safely and document accurately
- Coordinate care with interdisciplinary team members and family caregivers
- Educate patients and families about prognosis, symptom management and care options
- Maintain infection control practices and complete required screenings and documentation
- Complete Trinity skills checklist and participate in competency validation
- Travel between patient homes reliably and represent Holy Cross Home Care and Hospice professionally
Qualifications
- Active Registered Nurse license with ability to practice in Maryland, license verification required
- BLS certification required
- Experience or comfort working in long-term care, home health, or hospice settings preferred
- Ability to complete Trinity Skills Checklist and required competency testing
- Willingness to complete required health screenings and immunizations (see checklist)
- Successful completion of Trinity 10 panel drug screen and background checks
Knowledge & Skills
- Skilled in patient assessment, care planning and documentation in an electronic medical record
- Strong clinical judgment in symptom management and end-of-life care
- Excellent communication and family education skills
- Comfort with medication administration, wound care and basic procedures
- Time management and route planning for visiting multiple patient homes
- Sensitivity to cultural and psychosocial needs of hospice patients and families
