Roles & Responsibilities

• Provide compassionate, patient-centered hospice and long-term care in the home setting
• Perform thorough nursing assessments and develop individualized care plans
• Administer medications and treatments safely and document accurately
• Coordinate care with interdisciplinary team members and family caregivers
• Educate patients and families about prognosis, symptom management and care options
• Maintain infection control practices and complete required screenings and documentation
• Complete Trinity skills checklist and participate in competency validation
• Travel between patient homes reliably and represent Holy Cross Home Care and Hospice professionally

Qualifications

• Active Registered Nurse license with ability to practice in Maryland, license verification required
• BLS certification required
• Experience or comfort working in long-term care, home health, or hospice settings preferred
• Ability to complete Trinity Skills Checklist and required competency testing
• Willingness to complete required health screenings and immunizations (see checklist)
• Successful completion of Trinity 10 panel drug screen and background checks

Knowledge & Skills

• Skilled in patient assessment, care planning and documentation in an electronic medical record
• Strong clinical judgment in symptom management and end-of-life care
• Excellent communication and family education skills
• Comfort with medication administration, wound care and basic procedures
• Time management and route planning for visiting multiple patient homes
• Sensitivity to cultural and psychosocial needs of hospice patients and families
