Shift Type: Per diem / PRN - flexible schedule (day and night shifts as needed; rotating based on facility needs)

Roles & Responsibilities
- Provide compassionate, resident-centered nursing care in a long-term care setting
- Administer medications and treatments accurately and safely
- Perform nursing assessments and develop individualized care plans
- Document resident condition, care provided, and any changes in status thoroughly and timely
- Collaborate with the interdisciplinary team to coordinate care and improve outcomes
- Support residents with activities of daily living while promoting dignity and independence
- Respond to emergencies and perform BLS when required
- Participate in admissions, discharges, and care transitions with attention to continuity
Qualifications
- Current, unrestricted Registered Nurse (RN) license
- BLS certification required
- Experience in long-term care (LTC) preferred; comfort working with complex, chronic conditions
- Successful License/Certificate Verification as part of onboarding
- Completion of required personal health history items prior to assignment: Trinity color vision, 2-step PPD or IGRA (Quantiferon/T-Spot), COVID-19 vaccine, influenza vaccine, Trinity 10-panel drug screen, MMR, Hepatitis B, Varicella, physical/health statement, annual TB questionnaire, latex allergy questionnaire, fit test, and Tdap
Knowledge & Skills
- Strong resident assessment and clinical decision-making skills
- Medication administration and documentation best practices
- Knowledge of long-term care regulations and standards
- Effective communication with residents, families, and care teams
- Comfort using electronic health records and completing accurate charting
- Time management and prioritization in a fast-paced care environment
- Teamwork and the ability to mentor or support other staff
