Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in a long-term care setting
• Administer medications and treatments accurately and safely
• Perform nursing assessments and develop individualized care plans
• Document resident condition, care provided, and any changes in status thoroughly and timely
• Collaborate with the interdisciplinary team to coordinate care and improve outcomes
• Support residents with activities of daily living while promoting dignity and independence
• Respond to emergencies and perform BLS when required
• Participate in admissions, discharges, and care transitions with attention to continuity

Qualifications

• Current, unrestricted Registered Nurse (RN) license
• BLS certification required
• Experience in long-term care (LTC) preferred; comfort working with complex, chronic conditions
• Successful License/Certificate Verification as part of onboarding
• Completion of required personal health history items prior to assignment: Trinity color vision, 2-step PPD or IGRA (Quantiferon/T-Spot), COVID-19 vaccine, influenza vaccine, Trinity 10-panel drug screen, MMR, Hepatitis B, Varicella, physical/health statement, annual TB questionnaire, latex allergy questionnaire, fit test, and Tdap

Knowledge & Skills

• Strong resident assessment and clinical decision-making skills
• Medication administration and documentation best practices
• Knowledge of long-term care regulations and standards
• Effective communication with residents, families, and care teams
• Comfort using electronic health records and completing accurate charting
• Time management and prioritization in a fast-paced care environment
• Teamwork and the ability to mentor or support other staff
