Roles & Responsibilities

• Provide empathetic, high-quality nursing care to long-term care patients in their homes
• Assess patient health status and document findings clearly and promptly
• Implement individualized care plans and coordinate with interdisciplinary teams
• Administer medications and treatments safely while monitoring responses
• Educate patients and families on care routines, symptom management, and prevention
• Perform required competency tasks, skills checklists, and specialty testing
• Follow infection control protocols and maintain accurate clinical records

Qualifications

• Active Registered Nurse (RN) license in good standing
• BLS certification required
• NIHSS certification required
• Trinity Specialty Specific Competency Testing and Trinity Skills Checklist completed as required
• Current immunizations and health screenings (e.g., COVID-19 vaccine, Tdap, MMR, Hep B, TB testing, fit test, drug screen)
• Comfortable with home care and long-term care clinical environments
• Strong communication skills and a patient-centered approach

Education

• Graduate of an accredited nursing program (ADN or BSN preferred)
• Current RN license required

Knowledge & Skills

• Home care and long-term care clinical best practices
• Patient assessment, care planning, and documentation
• Medication administration and basic IV/site care knowledge
• Infection control, PPE use, and fit-tested respirator protocols
• Patient/family education and care coordination
• Time management, adaptability, and independent decision-making in home settings
