Roles & Responsibilities
- Provide empathetic, high-quality nursing care to long-term care patients in their homes
- Assess patient health status and document findings clearly and promptly
- Implement individualized care plans and coordinate with interdisciplinary teams
- Administer medications and treatments safely while monitoring responses
- Educate patients and families on care routines, symptom management, and prevention
- Perform required competency tasks, skills checklists, and specialty testing
- Follow infection control protocols and maintain accurate clinical records
Qualifications
- Active Registered Nurse (RN) license in good standing
- BLS certification required
- NIHSS certification required
- Trinity Specialty Specific Competency Testing and Trinity Skills Checklist completed as required
- Current immunizations and health screenings (e.g., COVID-19 vaccine, Tdap, MMR, Hep B, TB testing, fit test, drug screen)
- Comfortable with home care and long-term care clinical environments
- Strong communication skills and a patient-centered approach
Education
- Graduate of an accredited nursing program (ADN or BSN preferred)
- Current RN license required
Knowledge & Skills
- Home care and long-term care clinical best practices
- Patient assessment, care planning, and documentation
- Medication administration and basic IV/site care knowledge
- Infection control, PPE use, and fit-tested respirator protocols
- Patient/family education and care coordination
- Time management, adaptability, and independent decision-making in home settings
