Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in a long-term care setting
• Assess, monitor and document resident conditions and changes in status
• Administer medications and treatments safely and per facility policy
• Collaborate with interdisciplinary care teams to develop and update care plans
• Educate residents and families about care plans, medications and safety
• Maintain accurate and timely clinical documentation and charting
• Follow infection control practices and help maintain a safe environment
• Participate in competency testing and training to maintain clinical excellence

Qualifications

• Active Registered Nurse (RN) license — must be verifiable
• Current BLS (Basic Life Support) certification
• Ability to complete Trinity Specialty Specific Competency Testing
• Must be able to meet health and screening requirements: Fit Test; 2-step PPD or IGRA; Trinity 10 Panel Drug Screen; required immunizations including Influenza, MMR, Hepatitis B, Varicella, Tdap, COVID-19; physical/health statement; Trinity respirator evaluation and color vision testing
• Comfort working in long-term care environments and supporting residents with chronic needs

• Strong clinical assessment and critical thinking in long-term care
• Medication administration and documentation accuracy
• Infection control and prevention practices
• Clear, compassionate communication with residents, families and team members
• Time management, prioritization and adaptability in varied shift work
• Teamwork and collaboration with interdisciplinary care teams
