Shift Type: Per diem pool (PRN) — variable shifts; day, evening and night coverage as needed by the facility; exact start/end times and days vary based on assignment

Roles & Responsibilities
- Provide compassionate, resident-centered nursing care in a long-term care setting
- Assess, monitor and document resident conditions and changes in status
- Administer medications and treatments safely and per facility policy
- Collaborate with interdisciplinary care teams to develop and update care plans
- Educate residents and families about care plans, medications and safety
- Maintain accurate and timely clinical documentation and charting
- Follow infection control practices and help maintain a safe environment
- Participate in competency testing and training to maintain clinical excellence
Qualifications
- Active Registered Nurse (RN) license — must be verifiable
- Current BLS (Basic Life Support) certification
- Ability to complete Trinity Specialty Specific Competency Testing
- Must be able to meet health and screening requirements: Fit Test; 2-step PPD or IGRA; Trinity 10 Panel Drug Screen; required immunizations including Influenza, MMR, Hepatitis B, Varicella, Tdap, COVID-19; physical/health statement; Trinity respirator evaluation and color vision testing
- Comfort working in long-term care environments and supporting residents with chronic needs
Knowledge & Skills
- Strong clinical assessment and critical thinking in long-term care
- Medication administration and documentation accuracy
- Infection control and prevention practices
- Clear, compassionate communication with residents, families and team members
- Time management, prioritization and adaptability in varied shift work
- Teamwork and collaboration with interdisciplinary care teams
