Roles & Responsibilities

• Provide compassionate, high-quality nursing care to long-term care and home health patients in a per diem capacity
• Perform thorough patient assessments and collaborate on individualized care plans
• Administer medications and treatments safely and document care accurately
• Coordinate with interdisciplinary teams, families, and facility staff to optimize patient outcomes
• Maintain required certifications and complete Trinity specialty-specific competency testing
• Adhere to infection control standards and complete required health screenings and immunizations
• Complete timely and accurate documentation and license/certification verification

Qualifications

• Active Registered Nurse (RN) license (state of practice required)
• BLS (Basic Life Support) certification required
• License/certificate verification will be completed prior to start
• Willingness to complete Trinity specialty-specific competency testing
• Ability to pass required health screenings: fit test, TB testing (2-step PPD or IGRA), drug screen, and immunization requirements (MMR, Hep B, Varicella, Tdap, COVID-19, Influenza)
• Comfort working independently in home health and long-term care settings and collaborating with facility teams

Knowledge & Skills

• Solid knowledge of long-term care and home health nursing best practices
• Skilled in patient assessment, medication administration, and care planning
• Familiarity with infection prevention and immunization protocols
• Strong documentation skills and attention to detail
• Good communication and interpersonal skills for working with patients, families, and interdisciplinary teams
• Adaptability to varied assignment locations and patient populations
