Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications and treatments safely and on schedule
• Monitor vital signs and promptly report changes in condition
• Document care accurately in the medical record and communicate handoffs
• Collaborate with nurses, therapists, and providers to deliver coordinated care
• Educate patients and families on care plans and discharge instructions
• Support a positive, resident-centered environment that respects dignity

Qualifications

• Current, active LPN license in Pennsylvania
• Graduate of an accredited LPN/Vocational Nursing program
• Current BLS/CPR certification (or obtain upon hire)
• Comfortable with medication administration, charting, and basic wound care
• Strong communication skills and a teamwork mindset
• Previous experience in long-term care, skilled nursing, or similar setting preferred

Education

• Graduate of an accredited LPN/Vocational Nursing program
• High school diploma or GED

Knowledge & Skills

• Medication administration and documentation
• Vital signs assessment and patient observation
• Basic wound care and ADL assistance
• Electronic medical record (EMR) charting
• Patient/family education and care coordination
• Infection control and safety practices
