Roles & Responsibilities

• Provide compassionate daily care to residents, promoting dignity and independence
• Administer medications and treatments accurately and document per facility policy
• Monitor resident status and communicate changes to the care team promptly
• Assist with ADLs (bathing, dressing, mobility) while encouraging resident participation
• Collaborate with RNs, therapists, and support staff to implement individualized care plans
• Maintain clear, timely charting in the medical record to support continuity of care

Qualifications

• Current and active LPN license (must be valid for practice in Massachusetts)
• Completion of an accredited practical nursing program
• Basic Life Support (BLS) / CPR certification (current) preferred
• Previous experience in long-term care, rehab, or skilled nursing settings preferred
• Strong communication skills and ability to work effectively on a multidisciplinary team
• Ability to pass background checks and meet facility onboarding requirements

Education

• Completion of an accredited LPN / Practical Nursing program
• High school diploma or GED

Knowledge & Skills

• Medication administration and documentation best practices
• Wound care basics and skin integrity monitoring
• Familiarity with ADL support and mobility assistance techniques
• Competency with Electronic Medical Records and charting
• Infection control principles and safe patient handling
• Time management, prioritization, and calm decision-making under pressure
