Roles & Responsibilities

• Provide compassionate, person-centered care to residents, helping with activities of daily living
• Assist with bathing, dressing, toileting, transfers and mobility support with safety-first techniques
• Monitor and record vital signs and resident conditions, reporting changes to the clinical team
• Document care accurately and timely in resident charts and communicate with nursing staff
• Support a positive dining and social experience during meal times and activities
• Maintain infection control standards and follow facility policies to keep residents safe
• Collaborate with nurses, therapists and family members to promote resident well-being

Qualifications

• Current Licensed Nursing Assistant (LNA) credential (NH required; VT acceptable as noted)
• Completion of an approved LNA training program
• CPR/BLS certification preferred or willingness to obtain
• Ability to pass background check and meet facility immunization requirements
• Comfortable with resident lifting/transfer techniques and working as part of a team
• Strong communication skills and a caring, patient-centered attitude

Education

• High school diploma or GED preferred
• Successful completion of a state-approved LNA training program

Knowledge & Skills

• Skilled in personal care and activities of daily living
• Proficient in taking and recording vital signs
• Knowledge of infection prevention and safe patient handling
• Basic documentation and charting skills
• Ability to observe, report and escalate changes in resident condition
• Good interpersonal skills for working with residents, families and the care team
