Roles & Responsibilities

• Provide compassionate direct care to residents as a Licensed Practical Nurse
• Administer medications and treatments accurately and safely
• Perform timely and thorough clinical assessments and document in the chart
• Coordinate with the interdisciplinary team to deliver individualized care plans
• Monitor resident condition and communicate changes to nursing leadership and providers
• Support a positive, resident-centered environment through respectful communication and teamwork

Qualifications

• Active Licensed Practical Nurse (LPN) license (eligible/valid to practice in New Hampshire)
• Current Basic Life Support (BLS) certification preferred or willing to obtain
• Ability to pass facility-required health screenings and background checks
• Strong clinical judgment, reliability, and a compassionate bedside manner

Education

• Graduate of an accredited Practical Nursing program or equivalent LPN training

Knowledge & Skills

• Medication administration and safe documentation practices
• Basic assessment skills for adult and geriatric populations
• Clear communication with residents, families, and the care team
• Familiarity with electronic health records and clinical charting
• Ability to prioritize tasks and manage time during shift
