Roles & Responsibilities
- Provide compassionate direct patient care following plan of care and facility policies
- Administer medications and treatments safely and accurately
- Perform patient assessments, document findings, and communicate changes to the care team
- Assist with activities of daily living while promoting patient dignity and independence
- Collaborate with nurses, providers, and support staff to ensure coordinated care
- Maintain accurate, timely charting and handoffs to support continuity of care
- Practice infection control and safety procedures to protect patients and staff
Qualifications
- Current Licensed Practical Nurse (LPN) credential — must be eligible to practice in the state where you work
- Basic Life Support (BLS) / CPR certification (or willingness to obtain promptly)
- Comfortable administering medications, performing assessments, and documenting in the patient record
- Professional, compassionate communication skills and the ability to work as part of a multidisciplinary team
- Previous experience in long term care, rehabilitation, or similar settings is a plus
Education
- Completion of an accredited Practical Nursing program; High school diploma or GED required
Knowledge & Skills
- Strong clinical assessment and medication administration skills
- Knowledge of infection control and patient safety best practices
- Ability to prioritize tasks and manage time during a busy shift
- Good charting and documentation practices; attention to detail
- Teamwork, empathy, and clear communication with patients and families
