Roles & Responsibilities

• Provide compassionate direct patient care following plan of care and facility policies
• Administer medications and treatments safely and accurately
• Perform patient assessments, document findings, and communicate changes to the care team
• Assist with activities of daily living while promoting patient dignity and independence
• Collaborate with nurses, providers, and support staff to ensure coordinated care
• Maintain accurate, timely charting and handoffs to support continuity of care
• Practice infection control and safety procedures to protect patients and staff

Qualifications

• Current Licensed Practical Nurse (LPN) credential — must be eligible to practice in the state where you work
• Basic Life Support (BLS) / CPR certification (or willingness to obtain promptly)
• Comfortable administering medications, performing assessments, and documenting in the patient record
• Professional, compassionate communication skills and the ability to work as part of a multidisciplinary team
• Previous experience in long term care, rehabilitation, or similar settings is a plus

Education

• Completion of an accredited Practical Nursing program; High school diploma or GED required

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Knowledge of infection control and patient safety best practices
• Ability to prioritize tasks and manage time during a busy shift
• Good charting and documentation practices; attention to detail
• Teamwork, empathy, and clear communication with patients and families
