Roles & Responsibilities

• Provide direct patient care consistent with LPN scope of practice and facility protocols
• Administer medications, document accurately, and monitor patient responses
• Perform focused assessments and report changes promptly to RN/physician
• Collaborate with the interdisciplinary team to support patient-centered care plans
• Maintain clear, timely electronic health record documentation
• Support safe handoffs and transitions to ensure continuity of care

Qualifications

• Current Licensed Practical Nurse (LPN) license (or eligible to obtain NH licensure)
• Basic Life Support (BLS) certification required or willing to obtain
• Clinical experience in a long-term care or acute care setting preferred
• Ability to pass background check and meet facility onboarding requirements
• Strong communication skills and a patient-first attitude

Education

• Graduate of an accredited practical nursing program or equivalent diploma

Knowledge & Skills

• Medication administration and basic IV/shot competencies appropriate to LPN scope
• Patient assessment and vital signs monitoring
• Electronic health record documentation
• Infection control and safe patient handling
• Team collaboration and effective communication with patients and families
