Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients in the assigned unit
• Administer medications and treatments accurately and in a timely manner
• Monitor and document patient status, reporting any changes to the care team
• Collaborate with RNs, physicians and interdisciplinary staff to deliver coordinated care
• Educate patients and families about care plans, medications and discharge instructions
• Ensure compliance with facility policies, safety protocols and infection control practices

Qualifications

• Current LPN license in New Hampshire or compact license where applicable
• Active Basic Life Support (BLS) certification (or willingness to obtain prior to start)
• Clinical experience as an LPN preferred; recent hands-on experience caring for adult patients
• Up-to-date immunizations and ability to pass background and health screenings
• Strong communication skills and ability to work collaboratively in a team

Education

• Completion of an accredited LPN/LVN program
• High school diploma or equivalent

Knowledge & Skills

• Solid medication administration and documentation skills
• Familiarity with basic wound care and vital signs monitoring
• Comfortable using electronic medical records and point-of-care charting
• Good clinical judgment, time management and prioritization
• Patient-centered mindset with strong interpersonal skills
