Roles & Responsibilities

• Provide direct patient care with compassion and clinical competence
• Administer medications and treatments accurately and safely
• Monitor and document patient vitals, condition changes, and care provided
• Collaborate with RNs, physicians, and the care team to coordinate patient-centered care
• Educate patients and families about care plans, medications, and recovery
• Maintain a safe, clean environment and follow infection control protocols
• Contribute to accurate and timely charting and handoffs to ensure continuity of care

Qualifications

• Active Licensed Practical Nurse (LPN) licensure (must be valid in New Hampshire)
• Current BLS/CPR certification
• Comfortable administering medications and performing routine nursing procedures
• Strong communication skills and ability to work well on a multidisciplinary team
• Previous experience in long-term care or acute care settings preferred

Education

• Graduate of an accredited LPN/Vocational Nursing program

Knowledge & Skills

• Solid knowledge of basic nursing principles and medication administration
• Proficient in patient assessment and documenting clinical observations
• Skilled at time management and prioritizing patient needs during long shifts
• Able to use electronic health records for accurate charting
• Compassionate bedside manner and the ability to teach patients and families
