Candidate Pay Rate: $45 per hour

Roles & Responsibilities
- Assess adult and geriatric patients to identify speech, language, cognitive-communication and swallowing disorders
- Develop individualized treatment plans and deliver evidence-based therapy
- Collaborate with nursing, rehab staff, families and caregivers for coordinated care
- Document progress, update care plans and complete accurate billing records
- Provide patient and family education to support carryover and safe swallowing
- Participate in discharge planning and transitions of care to maximize outcomes
- Contribute to a positive facility culture by mentoring aides and supporting team goals
Qualifications
- Master’s degree in Speech-Language Pathology (required)
- State license to practice as an SLP in Colorado or ability to obtain licensure promptly
- ASHA CCC-SLP preferred or current certification in good standing
- Experience working in a Skilled Nursing Facility preferred; strong clinical skills with dysphagia and cognitive-communication disorders
- Strong documentation and electronic health record experience
- Excellent communication and teamwork skills; patient-centered attitude
Education
- Master’s degree in Speech-Language Pathology
Knowledge & Skills
- Clinical expertise in dysphagia assessment and management
- Skilled in cognitive-communication and language disorder treatment
- Familiarity with SNF workflows, rehab goals and interdisciplinary care
- Proficient with clinical documentation and productivity expectations
- Ability to educate and engage families and caregivers effectively
- Time management and caseload prioritization for consistent outcomes
