Roles & Responsibilities

• Assess and evaluate resident mobility, strength, balance and functional status to create person-centered treatment plans
• Develop and implement evidence-based therapeutic interventions that improve independence and quality of life
• Collaborate closely with nursing, OT, SLP and restorative teams to coordinate care and achieve measurable outcomes
• Document progress clearly and timely in the electronic medical record, ensuring regulatory compliance
• Educate residents and families on safe mobility, fall prevention and home exercise programs
• Supervise and mentor therapy assistants and support staff to maintain high-quality care
• Participate in interdisciplinary care meetings and contribute to discharge planning

Qualifications

• Current license to practice as a Physical Therapist (state licensure required; Colorado licensure or eligibility preferred)
• Degree from an accredited Physical Therapy program (DPT preferred)
• Basic Life Support (BLS/CPR) certification
• Experience in a Skilled Nursing Facility or post-acute care setting preferred
• Strong clinical reasoning, documentation skills, and ability to work collaboratively
• Able to safely lift/assist patients as required and follow facility safety protocols

Education

• Graduation from an accredited Physical Therapy program (Doctor of Physical Therapy (DPT) preferred)

Knowledge & Skills

• Skilled in geriatric rehabilitation, transfers, gait training and balance retraining
• Familiarity with restorative nursing principles and SNF workflows
• Knowledge of Medicare/Medicaid documentation requirements and compliance
• Proficient with patient assessment tools and outcome measures
• Excellent communication and patient/family education skills
• Comfortable using electronic medical records and completing timely documentation
