Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a skilled nursing setting
• Administer medications and treatments accurately and document care in the resident record
• Monitor resident condition, report changes to the care team, and participate in care planning
• Assist with activities of daily living while promoting independence and dignity
• Collaborate with nurses, therapists, and support staff to deliver coordinated care
• Maintain clean, safe patient environments and follow infection control practices
• Communicate with families and caregivers in a respectful and informative manner

Qualifications

• Active Licensed Practical Nurse (LPN) license in Massachusetts required
• Current BLS/CPR certification required or must obtain upon hire
• Previous experience in long-term care or skilled nursing preferred
• Ability to pass background check and meet facility credentialing requirements
• Strong interpersonal skills and a patient-centered approach

Education

• Graduate of an accredited Practical Nursing (LPN) program

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and infection control procedures
• Comfortable with electronic medical records and charting
• Skilled in resident assessment and recognizing changes in condition
• Strong teamwork, communication, and time management abilities
