Roles & Responsibilities

• Provide compassionate direct patient care and assist with activities of daily living
• Take and record vital signs accurately and report changes to the nursing team
• Support safe mobility and transfers, ensuring patient comfort and fall prevention
• Assist with feeding, hygiene, and toileting while preserving patient dignity
• Communicate observations and concerns to nurses and interdisciplinary team members
• Maintain a clean, safe environment and follow infection control protocols
• Document care delivered clearly and timely to support continuity of care

Qualifications

• Current CNA certification in the state of Pennsylvania (required)
• Previous experience in long-term care or skilled nursing settings is a plus
• Ability to pass background checks and meet facility onboarding requirements
• Strong communication skills and a patient-centered attitude

Education

• High school diploma or GED preferred
• Completed state-approved CNA training program and active CNA certification

Knowledge & Skills

• Comfortable with basic clinical tasks: vital signs, ADL assistance, patient transfers
• Knowledge of infection prevention and standard precautions
• Good observational skills and the ability to escalate patient changes
• Team-oriented with reliable time management and documentation habits
• Customer service mindset when interacting with patients and families
